Employee discount still not applied!! Becoming a pain!

I have been trying to get my employee discount applied since February 25, 2014. It is now June 2, 2014 and my discount is still not applied. I have been given the runaround for almost 3 months. Before I made the switch to T-mobile I called customer service several times to see if I qualified for my discount. I have submitted the required documentation about 5 times and somehow no one has any record of my inquiry. I called customer service, the employment advantage plan, and talked to several people in the store all who have given me different answers. I have received several email confirmation stating the below. When I called customer service they stated that i needed some type of code that was never sent.

 

So my question is, when will my discount get applied or do i need to leave t--mobile again to save more money?
