ETF problems Chat reps no help. Do problems get lost and never taken care of?

After changing over three of my five lines with Verizon and and ordering two phones I never received an RMA to send the old phones in to be repaid for the ETF which two of the lines incurred (one line was already off contract). Well within the allotted time frame I got on the online chat to ask what happened to the RMA. The chat representative said it would be looked into I would be gotten back to within 72 hours. I was called but only to be told that they were still looking into it. Several weeks passed and nothing from T-mobile. Again I got on the chat and again explained what happened, as if no recored of my first attempt was recorded, and was told that this would be taken to a manager and I would hear back. After several more weeks nothing from them. Did this just get "lost in the mail" or is there a problem with the chat reps following through?
