most users who have switched to t-mobile have done that in a store. you would have waited 6-8 weeks and got your master card but if you are among the unlucky few who tried this online, i can tell you it DOES NOT WORK. here's why. the online switch over generates an RMA form with a mailing label that asks you to send your old phone (qualifying device) to a PO BOX for Assurant Insurance. This means that instead of being treated as an ETF reimbursement, it gets logged as a RETURN and that's where the problem begins. T-Mobile has no way of tracing the phone either by IMEI or RMA id which means if you get a denial on ETF for this device, you are out of luck. now you'd ask why would anyone do online switch over if you can goto a store and hand them your device - 1. Dumb enough to think online is more convenient and that it would work :-) 2. you have a second line used by family member who is not in the same vicinity as you are or not close to a t-mobile store.
so in a nutshell - if you took your device to the store - great ! online - you risk losing $200-$250 of ETF reimbursement !
