Dispute a charge

I was charged for downloading an app on my account. However, I haven't done anything of the sort. This happened when i first started my contract about 3 years ago, but back then they put a block on the account so this couldn't happen again.  That block must have been lost at some point.  I don't want to be charged for something I didn't purchase. How do I dispute the charge?
