PBTD charges are unsupported and T-Mobile doesn't explain them

I have a $3 PBTD plan. I have been (and I am still)  out of the country at least for the last 30 days.

I have bought a local SIM card and stored my T-Mobile SIM card in a box for the same period. There is no way, absolutely no way that the phone may have been used, but when I check my T-Mobile account on the web, I see that every now and then I am charged $3.

I have contacted customer support thru the web and even though they confirm that with SIM card out of the phone there can be no charge, their answer is turning around the burden of proof : "If there are charges it is because there has been phone usage" and as PBTD plans have no activity log, they cannot tell me what was the activity and I am being skimmed off every now and then of $3 and with no option. I have seen several posts describing that they have also been charged with the phone off.

I believe that before assuming directly that T-Mobile rips us off every now and then of small change, we must explore other rational alternatives.

1. Does anyone know of any situation where a PBTD plan might get a charge with the phone off and the SIM card out of the phone ? I am imagining a voice mail, or a text message, or using the T-Mobile website and logging in or whatever it may be.

2. If there is no such a situation, how can I fight this within T-Mobile ?

Any help will be appreciated. Thanks.
